Accident Investigation & Reporting

Reminder: Effective January 1, 2002, the forms and requirements for recording and reporting occupational injuries and illnesses have changed. While some of these videos may still refer to previous practices, please ensure that you are informed of the new reporting requirements. If you are in doubt, feel free to contact the consultation program office or the OSHA area office.

Accident Investigation (MAN-001)
This video was developed by Boise Cascade Corporation. It deals with the procedures that should be followed after an accident has occurred.  It is directed to the foreman or the first-line supervisor who are responsible for hazard investigations. Although the plant's safety director may assist in accident follow-up or hazard identification in the workplace, the foreman and first-line supervisor must become involved.  It is essential that they have knowledge about the work area, the machinery, and their crew's work habits. Accident Investigation (MAN-002)
Much can be learned from success­ful completion of investigations to improve procedures and safety rules, and prevent accidents and injuries in the future. The key result from any investigation should be to prevent a recurrence of the same or similar accident or incident. This program includes:  types of accidents, outcomes of accidents, contributory factors leading to accidents, composition of investigation team, strategy of the investigation, and benefits of accident investigations. Incident Reporting: You Can Save a Life (MAN‑014)
Using an “It’s A Wonderful Life” theme, this powerful video program teaches workers the importance of following acceptable safety standards, controlling hazards and reporting unsafe work conditions – before incidents become accidents. Documentation of Safety Efforts (MAN-006)
Teaches supervisors (and employees) the importance of providing written  documentation for all training and safety efforts.  More regulatory and legal agencies are requiring written documentation to "prove" that training and safety efforts are accomplished.  This program outlines the standards and how to meet these requirements. Accident Investigation in the Workplace (MAN‑049K) 1
Supervisor Training And Employee Awareness. This program was specially created to help your supervisors develop first-rate accident investigation skills and help your employees understand how their cooperation can prevent future accidents from happening. For supervisors, the program includes a workbook and video that explain in detail how to find the root cause of accidents. They show how to take control of an accident scene, how to collect evidence, how to interview witnesses, how to take corrective action, and explain other vital investigative techniques. A separate video and workbook for employees encourages them to open up -- not clam up -- during investigations. The video explains in convincing fashion that the objective is to prevent further accidents — a goal that's in their interest, too. Recordkeeping: True to Form (MAN-050K) 1
Due to criticisms in paperwork, reporting issues and interpretation of terms, OSHA recently revised and simplified it's recordkeeping system. Coastal's new video provides an important overview of the changes to safety directors and managers, safety committee members, human resources managers, or anyone responsible for safety and insurance paperwork. Risk = Hazard + Outrage: A Formula for Effective Risk Communication (MAN‑055K) 1 q
This Peter Sandman video, designed for anyone concerned with the public''s perception of risk in the workplace, community, or environment, presents useful and practical information for developing a companywide risk communication strategy. Topics include the 20 components of outrage that determine how people will react to a risk and practical suggestions for preventing or minimizing each component; the four stages of risk communication within an organization; and the complexities of risk communication. Job Safety Analysis (MAN‑053)

Target Zero: Pro-Active Safety Attitudes Kit (MAN‑039K)
Zero accidents in the workplace is a possibility – and it all starts with a vision. Astronaut Buzz Aldrin helps to emphasize the importance of pro-active safety by reminding us that there was a time when the possible seemed impossible. Mine Safety and You: The Human Element (MAN‑020)
The general causes of accidents in the mining industry have been blamed on three factors: conditions, machines and the human factor. Of all those accidents the human factor is the cause of 80% of all mishaps. This video is a five segment production and identifies three basic safety types. Knowing your type and working with these characteristics can reduce your accident profile. At the beginning of the video is the 18-minute full documentary covering accident causes and the human elements of safety. Following this documentary are four shorter video segments. Each complements a color-coded section of the trainer's guide. Featuring Florida State head football coach Bobby Bowden, this training tool shows supervisors and employers how to actively support a safe workplace. Even if you're not a sports fan, you'll enjoy Coach Bowden as he shares how successful coaching techniques can result in safe work habits. The video is based on the book, The Safety Coach: Unleash the 7 C's for World-Class Safety Performance!, written by David Sarkus, MS, CSP, who also serves as on-camera host of the video. Safety and the Young Worker (MAN‑029)
This video looks at the special problems often experienced by young workers on the job.  Through a series of dramatic re-enactments, the video shows unsafe work situations and how to correct them.  Through interviews with four injured workers, it also shows the tragic consequences of workplace accidents. This video is young people talking to other young people about safety and about that first job.  It is primarily directed at teenage audiences, but is also suitable for general viewing. The Troubled Employee at Work (MAN-041)
A supervisor needs to know their employees and their limitations. Communication and counseling should be provided as an employee's performance decreased.
